Submit the essentials here, and staff will follow up on fit, intake, and next steps. The goal is the same as yours: the right support, reached quickly, with the person’s dignity intact.
Referral partners
Health and social service providers, schools, courts and legal partners, probation and corrections, treatment providers, employers, and community agencies all refer to Hope Council — for assessment, diversion programming, peer support, family services, testing, and navigation. You stay in the loop — where releases allow
Confidentiality laws govern what can flow back to referrers. With releases in place, staff can confirm connection and coordinate; without them, you may hear less — that’s the law working, not the referral failing.
