This form registers you for the Underage Drug/Alcohol Accountability Program after an underage alcohol or other-drug citation. Your citation determines your class: the 8-hour alcohol class ($175) or the 12-hour other-drug class ($265). You must attach a copy of your citation to receive court credit, and you’ll receive confirmation of enrollment by email.

Fees are $175 (8-hour) or $265 (12-hour), payable to Hope Council. Online payment is being added in a later phase; until then, call 262-658-8166 or mail a check to 6103 39th Ave., Kenosha, WI 53142.
